Uttarbasti is advised to administer through uttarmarga of Niruha basti marga that is urethra/vagina.

It has very good qualities and it has to administer after Niruha basti , therefore it is called Uttarbasti

*Indications of Uttarbasti:-

1)Uttarbasti pacifies Vata ,it helps to conceive  , useful in infertility.च.सं.सि.9/63-65

2)It is useful in vaginal prolapse with severe pelvic pain.

3)Useful in  all Yoniyapad  disorders, menorrhagia,menometrorrhagia, amenorrhea,dismenorrhoea,all menstrual disorders , history of habitual abortions, history of fetal loss etc.

*-Time For Performing Uttarbasti:-

1) Uttarbasti through vaginal route should be given during Rutukala(proliferative phase),because during this period uterus and vagina are free from coverings and orifices are open, therefore it receive madicine easily .

2)During Rutukala Yoni is the Apavritta (open).  Means size of either introitus or os of  cervix does not increase,’open’refers to functional aspect and not anotomical.

After Rutukala Yoni is apavrutta,cervical mucous becomes sticky and thick therefore Uttarbasti should be avoided.

3)After two or three Asthapan basti , Uttarbasti is given for three to five consecutive days for at least three consecutive cycles.

3) According to Charakacharya ,Uttarbasti has to be given two or three times a day for three consecutive days.After a gap of three days it should be repeated in the same manner but with increased amount of medicine.

4) According to Sushrutacharya Uttarbasti can  be continue for 4days

6)But use of Uttarbasti is not restricted to Rutukala only,if patient’s condition demands it can apply at any time after preparation.

*Dose of Medicines (Dravya Matra):

1)Oil based medicine one Prasruta/prastha(12 ml approximately) to pacify dosha in the uterus

If strength of disease and patient is less then quantity should be half or less,to be decided by the physician.

To remove dosha(Shodhana) from the uterus, oil based medicine should be

double 

2)If decoction is used to remove dosha from the uterus ,the quantity should be two Prastha, approximately 24ml

3) According to Vagbhata dose is one Prakuncha (48ml approximately),and he advised to increase gradually.अ.सं.सू.28/73अ.हृ.सू.19/80

*Method of administration of Uttarbasti through vaginal route:

1)The woman should be placed in supine position with thighs flexed and elevated knees.

2)The cannula should be inserted in the vaginal passage slowly with steady hand, following the direction of the passage.

*Complications of Uttarbasti:-

*Uttarbasti prattyagaman Kala:-Time for expulsion of Medicines

According to Acharya Gayadasa and Dridhbala it is 100matra that is approximately 90seconds.

*Contraindications of Uttarbasti:-******

The Uttarbasti is not given through vaginal passage of Bala(age below 16) as it is narrow  च.सं.सि.9/67चक्र.टीका

*PRACTICALLY WE DO UTTARBASTI IN THE FOLLOWING WAYS   :

1) Uttarbasti medicine upto the level of cervix ,that is into the vagina only-(48)ml

2)Small quantity of medicine 3 ml to 5ml into the uterine cavity and remaining in the vagina –(48ml)

3)Medicine insert through the cervix 9-10ml which will pass through cervix-uterus-fallopian tubes and spill in the pelvic cavity and remaining in the vagina- (48ml)

*Uttarbasti should administer in such a way where there is demands of the condition of diseases.
